Tell me all about your dog.

This is where we really begin. Before we meet, I want to get to know your dog properly: who they are, what everyday life looks like, and what you are hoping for. It takes about ten to fifteen minutes with a cuppa. There are no wrong answers, "not sure" is a perfectly good answer, and the honest version is always more useful than the tidy one. I read every word before we meet.

If life interrupts, your answers save themselves on this device and will be here when you get back. If I'm working with more than one of your dogs, send me the form once for each dog.

Hi, I'm Laura from Green Dogs. Thank you for booking a behaviour consultation with me. The more information I have about you and your dog, the easier it is for me to help you. Information is power, so don't worry about getting things wrong and don't worry about any judgement, just write things down as you feel and see them. When I come to see you in person we'll chat through everything again, so give me as much information as you can, and I really look forward to seeing you soon. Bye for now.

Before we start

This form is a welcome, not paperwork. The sections below cover you, your dog, health, everyday life, the story so far, a couple of honest safety questions and a few practical bits. Blank optionals are absolutely fine, and anything marked with a is one I really need. Pop your dog's name in early on and the form will start talking about them properly.
